Because Healthcare Should Feel Human Again ...
I’ve also walked this road personally. I know what it’s like to fight for my own health. And I know the heartbreak and frustration of trying to coordinate care for someone I love from hundreds of miles away — my own mother, who lives with chronic illness in New York.
Those experiences opened my eyes to the gaps in our healthcare system. Hospitals patch patients up and send them home, but too often they return — not because they failed, but because the system did. Most readmissions could be prevented with better communication, proactive monitoring, and ongoing support after discharge.
Thrive was created to fill that gap.
We exist to make healthcare feel personal again — to bring professional, high-touch support into the home and provide patients with the tools, guidance, and partnership they need to truly thrive. Our approach doesn’t replace existing care; it enhances it. We connect the dots between providers, patients, and families so that every person we serve can live with greater freedom, confidence, and dignity.
Our Mission
To deliver compassionate, nurse-led care at home — reducing readmissions, enriching health, and helping people thrive where they belong.
Our Values
-
Compassion: Every patient is a person first — deserving of empathy, respect, and care tailored to their life.
-
Collaboration: We partner with families, providers, facilities, and hospitals to create a true circle of support.
-
Empowerment: We help patients make informed choices, take ownership of their health, and stay independent.
-
Trust: We keep our promises. We show up. We do what we say — because continuity of care starts with consistency.
Because people don’t always need more medicine or more appointments — they need more connection, support, and care where it matters most: home.
Sheron McFarlane, RN — Founder & CEO of Thrive InHome Care Solutions
“Nurse. Daughter. Advocate. Believer that care should always feel human.”

Thrive InHome Care Solutions was born from a simple but powerful belief: healthcare should be more than medications, appointments, and discharge instructions — it should feel human.
As a registered nurse with over a decade of experience in clinical care, care management, and patient advocacy, I’ve seen too many people sent home with instructions but no real plan. Too many families doing their best but still feeling lost. Too many patients wishing for less hassle — and more help.
