• Have one or more chronic conditions (e.g., diabetes, CHF, COPD, hypertension, CKD)

• Are high risk for readmission or live alone

• Need help with medication adherence, vitals tracking, or health education

• Recently discharged from hospital, rehab, or skilled nursing care

• Prefer ongoing care without daily in-home visits

Our team provides Medicare-covered RPM and CCM services, bridging the gap between visits with real connection and clinical oversight.

You can also fax referrals to (772) 492-4342 using our HIPAA-secure cover sheet.

What Happens Next:​

• Once your referral is received:

• Confirmation: You’ll receive a confirmation text (patient initials only).

• Verification: We verify insurance eligibility within 1 business day.

• Contact: Our nurse team calls the patient to confirm consent and start date.

• Device Delivery: RPM devices are typically delivered within 24–48 hours.

• Start of Care (SOC): Scheduled within 1 business day of patient readiness.
